Care Management at Lifestages

Lifestages provides exceptional, compassionate care for individuals and families facing new or advancing health challenges.  We are your partner in care - providing you with the support, information, and resources you need at every stage in life.

 

With over 25 years of experience in the field, Lisa Donnellan, LICSW is Lifestage's lead geriatric care manager.  She helps you and your family identify strengths and needs, form a long-term care plan, and connect with trusted services.  She provides support with medical appointments to advocate for the care and resources you need to make your daily life easier.  Care management can be especially helpful when family members live far apart. 

 

Care is provided in the home and should not be booked online. Please contact the office to schedule a consultation call.

We provide support with medical appointments to help advocate for the care you and your loved one needs.

Adjusting to the changes and challenges of aging isn’t always easy. Sometimes, we need support transitioning through major life changes, maintaining safety. Your care manager can keep the family ‘in the loop’ and provide coaching for dealing with emotionally sensitive issues.
